Start by creating a rounded rectangle using rounded rectangle shape tool.

Press Ctrl+T. Right click the layer and select perspective. Adjust nodes to get the shape as shown.
Enable add to shape area at the top bar and create a vertical rectangle using rectangular shape tool.

Make sure add to shape area is still enabled at the top bar and create a horizontal rectangle using rectangular shape tool.
Press Ctrl+T. Right click the layer and select perspective. Adjust nodes to get the shape as shown.
Place the shape on the top of the flask.
Select all the paths using direct selection tool and click combine icon at the top bar. It will merge the three shapes together as one. Double click the layer to open layer style window. Apply layer style with the settings shown.

Ctrl+Click the layer thumbnail to get the selection.
From select menu apply modify>>contract.
Create a new layer. Fill it with white color.
Select the eraser tool and with a big feathered brush settings remove the area of right side.

 
Select both the layers. Press Ctrl+T and rotate.
Duplicate the main flask layer. Move it above all layers in layer palette. Remove layer style.
Enable subtract form shape area form the top bar.
Using pen tool create a shape as shown in the image.
Double click the layer to open layer style window. Apply layer style with the settings shown.
From select menu apply modify>>contract.
Create a new layer. Fill it with white to black linear gradient.
Change the layer mode to screen.
Create a rain drop shape using custom shape tool.
Double click the layer to open layer style window. Apply layer style with the settings shown.
Select and merge all the layers. Double click the layer to open layer style window. Apply layer style with the settings shown.
