Creating Windows XP icon for medical assistance

Start by creating an ellipse using elliptical shape tool.

Press Ctrl+T. Right click the layer and select warp. Adjust nodes to get the shape as shown.

Enable subtract form shape area form the top bar.

Using pen tool create a distorted rectangular shape.

Double click the layer to open layer style window. Apply layer style with the settings shown.

Duplicate layer, flip horizontal, scale down and place as shown. Double click the layer to open layer style window. Reverse the angle of gradient overlay.

Using polygon shape tool create a triangle. Press Ctrl+T. Right click the layer and select warp. Adjust nodes to get the shape as shown.

Create a rectangle for collar using rectangular shape tool. Press Ctrl+T. Rotate it. Right click the layer and select warp. Adjust nodes to get the shape as shown.

Apply the same layer style as before. Double click the layer to open layer style window. Apply drop shadow layer style with the settings shown.

Duplicate the layer scale down and flip horizontal. Place as shown.

Create an ellipse for the face using elliptical shape tool.

Double click the layer to open layer style window. Apply layer style with the settings shown.

Create another ellipse for the hair using elliptical shape tool. Press Ctrl+T. Right click the layer and select warp. Adjust nodes to get the shape as shown. Move this layer below all the layers in layer order in layer palette.

Select the face ellipse path using direct selection tool. Using add anchor point tool add some points as shown.

Adjust the nodes to get the shape as shown.

Create a rectangle using rectangular shape tool. Press Ctrl+T. Right click the layer and select warp. Adjust nodes to get the shape as shown.

Create a horizontal rectangular line using rectangular shape tool. Select the path using path selection tool. Duplicate the path. Press Ctrl+T. Right click the layer and select rotate 90°.
